The NHS want to hear from people who use Patient Transport, as well as relatives or carers who support someone who uses the service.

Patient Transport helps eligible people travel to and from healthcare appointments and settings, when they cannot use other forms of transport because of their medical or mobility needs.

This short survey will help them to understand people’s experiences of using Patient Transport, including what works well, any difficulties they face and what could be improved.

Your feedback will help them to understand what matters most to people using the service and where improvements could be made.
