Patient Transport helps eligible people travel to and from healthcare appointments and settings, when they cannot use other forms of transport because of their medical or mobility needs.
This short survey will help the NHS understand people’s experiences of using Patient Transport, including what works well, any difficulties they face and what could be improved.
Your feedback will help them to understand what matters most to people using the service and where improvements could be made.

The survey closes on Monday 2 November 2026 at 5:00pm
